Endoscopic mitral valve repair has advantages such as excellent exposure of details of mitral valve, faster postoperative recovery and cosmetic benefits. However, it is technically challenging and has many pitfalls. There are many technical tricks and modifications that surgeons should know to achieve durable outcomes without complications. This article introduces the theoretical rationales and technical details of the approach, exposure and valve repair techniques in endoscopic mitral valve repair.
