The management of patients with chronic aortic insufficiency is an important clinical problem someway still open. A correct understanding of the patient should take into account the pathophysiology and natural history of the disease, as well as the results that can be achieved with medical and surgical therapy. This review will focus on the clinical and instrumental parameters that may bring to the best therapeutic decision for the individual patient. Definitely the management of patients with chronic aortic insufficiency consists mainly in the definition of the right time to surgery with the lowest risk and the greatest benefit.
