PURPOSE: To analyse the visual performance, optical quality, and patient satisfaction following bilateral implantation of a new intraocular lens (IOL) with an spiral optic design.

METHODS: Prospective, observational, non-comparative study including a total of 98 eyes of 49 patients (age range: 43-79 years). All eyes underwent uneventful bilateral cataract surgery with the implantation of the Galaxy IOL (Rayner®, Worthing, UK). Visual acuity (VA), refraction, mesopic and photopic contrast sensitivity (CS), defocus curve, simulated halos and glare perception, and patient-reported outcomes (Catquest 9SF questionnaire) were evaluated during a 3-month follow-up.

RESULTS: Mean 3-month postoperative spherical equivalent and cylinder were - 0.06 ± 0.26 and - 0.12 ± 0.25 D. Mean binocular 3-monh postoperative logMAR corrected distance VA was - 0.08 ± 0.06, and mean 3-month binocular distance-corrected intermediate (66 cm) and near (40 cm) VAs were - 0.06 ± 0.06 and 0.11 ± 0.08 logMAR, respectively. The binocular defocus curve at 1 and 3 months showed functional vision (≤ 0.2 logMAR) from + 1.00 D to -2.50 D. Mesopic and photopic CS showed consistent improvement between 1 and 3 months almost across all spatial frequencies (p ≤ 0.035). Likewise, a marked reduction in halo and glare phenomena was observed from 1 to 3 months postoperatively (p < 0.001). Rasch Catquest scores showed a significant reduction in self-reported visual disability at both 1 month and 3 months postoperatively compared to baseline (p < 0.001).

CONCLUSIONS: The spiral optic-based IOL provides a continuous full range of vision with optimized CS and a controlled incidence of visual disturbances, leading to high levels of patient satisfaction.
