Аннотация

IMAGE(S): Cardiac papillary fibroelastoma (CPFE) is pictured on transthoracic echocardiogram. CPFEs are rare cardiac masses associated with myocardial infarction, syncope, arterial embolism, and sudden death.

CASE SUMMARY: An 87-year-old woman with paroxysmal atrial fibrillation presented for Watchman evaluation and Cox-Maze procedure. A preprocedural transthoracic echocardiogram demonstrated a 1.2-cm CPFE in her left atrium. Cardiothoracic surgery performed a combined excision of the left CPFE and surgical management of atrial fibrillation. Postoperatively, she developed cardiogenic shock and atrial fibrillation recurrence.

TAKE-HOME MESSAGES: The left atrium is an uncommon spot for CPFE and clinicians should use caution when considering these patients for combined Cox-Maze procedure with CPFE excision given theoretical risk of atrial fibrillation with manipulation of the left atrium. Patients may benefit from staged procedures, hypothetically reducing the risk of postoperative atrial fibrillation.
