OBJECTIVE: To analyze treatment outcomes in patients with perforated gastroduodenal ulcer in the Ryazan region between 2019 and 2024.

MATERIAL AND METHODS: We analyzed primary statistical data from the annual reports of the chief surgeon of the Ryazan region in 2019-2024.

RESULTS: There were 1.324 patients with perforated gastroduodenal ulcer. Of these, 562 patients admitted later than 24 hours after manifestation. Surgical interventions were performed in 1.242 patients; 941 patients underwent open surgery. In total, 172 patients died after surgical interventions. The total number of deaths was 237. The highest number of cases occurred in 2020 and 2021; these years also had the highest mortality and the highest number of patients with delayed hospitalization. The smallest number of patients was noted in 2024. Laparoscopic intervention was performed in 301 patients over the past 6 years. The largest number of interventions were performed in 2021, the smallest number - in 2024. Fourteen patients admitted later than 24 hours after manifestation and underwent laparoscopic interventions. Of these, more than half occurred in 2021. In 2019, 2022 and 2024, patients with late hospitalization did not undergo surgery through laparoscopic access. There was no postoperative mortality after laparoscopic surgery over the past 6 years.

CONCLUSION: The number of patients with perforated ulcers clearly decreased, but mortality rate increases over time. Over the past six years, the number of laparoscopic interventions for perforated ulcers decreased in the region. This requires updating laparoscopic equipment, as well as improving the laparoscopic skills in doctors. Peak morbidity and mortality occurred during the COVID-19 pandemic.
