• Only one case has been reported of an appendiceal neoplasm with a thigh abscess. • The classic presentation of appendicular neoplasms includes appendicitis-like symptoms. • We performed a right hemicolectomy and thigh abscess drainage. • No guidelines exist but multidisciplinary management leads to better outcomes.
