Аннотация

Surgery is crucial in managing early-stage cervical cancer. At present, radical hysterectomy with bilateral pelvic lymph node dissection with pelvic autonomic nerve preservation (type C1 surgery) is the primary surgical procedure to reduce postoperative dysfunction of urination, defecation and sexual function. To minimize the impact of surgery on female pelvic floor function and alleviate postoperative pelvic floor dysfunction, anatomical reconstruction of pelvic structures was performed based on the anatomical characteristics of type C1 surgery. Referring to the surgical methods of prostate cancer, the primary process of anatomical reconstruction of pelvic structure with type C1 surgery is as follows: first, after eliminating the sacral ligament at the rectal level, uterosacral ligament suspension was performed to strengthen the vaginal apex support. Second, the vesicocervical ligaments were reconstructed by restoring the triangular structure of the bladder based on the suspended vagina and the position of the vesicocervical ligament stump. The pelvic floor structure was reconstructed successfully after radical surgery for early-stage cervical cancer, per the modified surgical method demonstrated in the video. Our improved surgical method successfully reconstructed the pelvic floor structure of female based on a radical hysterectomy completion, which will be conducive to alleviating pelvic floor dysfunction and improving the quality of patients' lives after surgery.
