When a new phenomenon is reported, despite many previous reports on larger cohorts of patients, one naturally asks why the finding is novel. Is this a new phenomenon, or merely identified because one is looking carefully for it for the first time? This commentary explores possible reasons for the novel finding of essential thrombocythemia emerging in a cohort of Nucleophosmin 1 (NPM1)-mutated acute myeloid leukaemia (AML) patients.
