Аннотация

BACKGROUND: Point-of-care ultrasound (POCUS) has rapidly evolved as a transformative bedside imaging modality in respiratory medicine. Unlike conventional radiological investigations, POCUS provides real-time, dynamic assessment of pulmonary and pleural pathology without exposure to ionizing radiation. Its increasing adoption across emergency, critical care, and outpatient settings reflects its clinical versatility and diagnostic accuracy. In recent years, POCUS has gained particular importance in resource-limited environments, where access to advanced imaging modalities such as computed tomography may be restricted.

OBJECTIVE: To comprehensively review the role, applications, diagnostic utility, and limitations of POCUS in respiratory medicine, with emphasis on its integration into routine clinical practice.

METHODOLOGY: A narrative review was conducted through a structured search of electronic databases, including PubMed, Scopus, and Google Scholar. Literature published in English was identified using keywords such as "point-of-care ultrasound," "lung ultrasound," "respiratory diseases," "pleural disorders," and "bedside imaging." Relevant clinical studies, guidelines, systematic reviews, and narrative reviews focusing on diagnostic and procedural applications of POCUS were included. Articles lacking clinical relevance or focusing exclusively on nonrespiratory applications were excluded. The selected literature was synthesized qualitatively to highlight key diagnostic principles, clinical applications, and practical considerations.

RESULTS: POCUS has demonstrated high diagnostic accuracy in a wide range of respiratory conditions, including pleural effusion, pneumothorax, pneumonia, interstitial lung disease, and pulmonary edema. It allows rapid differentiation of causes of acute respiratory failure and facilitates early clinical decision-making. Lung ultrasound artifacts such as A-lines, B-lines, and consolidation patterns provide important diagnostic clues. POCUS also plays a crucial role in guiding procedures such as thoracentesis, thereby improving safety and reducing complications. In comparison to chest radiography, it offers superior sensitivity in detecting pleural and parenchymal abnormalities, particularly in critically ill patients. Despite its advantages, limitations include operator dependency, a learning curve, and challenges in standardization across different clinical settings.

CONCLUSION: POCUS is an indispensable and rapidly expanding tool in respiratory medicine, offering a safe, cost-effective, and efficient alternative to conventional imaging. Its integration into clinical practice enhances diagnostic precision, supports bedside decision-making, and improves procedural safety. Wider adoption, supported by structured training and standardized protocols, is essential to fully realize its potential in both high-resource and resource-constrained settings.
