Аннотация

BACKGROUND

Contemporary aesthetic surgery planning increasingly relies on evidence-based frameworks, including the upper-to-lower pole proportion of 45:55. Critics argue this template reflects transient social media aesthetics rather than stable biological preferences. Whether Western classical sculpture-which historically informed surgical training-demonstrates proportions consistent with contemporary frameworks remains unquantified.

OBJECTIVES

To quantify breast proportions in Western classical sculpture across five artistic eras and contextualize contemporary aesthetic frameworks within historical artistic tradition.

METHODS

From 122 sculptures spanning Hellenistic through Neoclassical eras, we extracted three-dimensional measurements using validated photogrammetric methods. Upper-to-lower pole ratios were computed from footprint measurements. Bilateral asymmetry was analyzed to assess biological realism. Measurements were normalized to 165 cm for clinical comparability.

RESULTS

Median upper-to-lower pole ratio was 47.6% [IQR 42.7-53.0], with 30.6% falling within 45 ± 3%. Era-specific medians ranged from 43.7% (Baroque) to 49.7% (Renaissance), with 80% of eras falling between 45 and 50%. Human-scaled measurements: sternal notch-to-nipple 18.5 cm (left), 18.2 cm (right); projection 7.1 cm. Consistent directional asymmetry (left > right) mirrored clinical populations (p = 0.009), demonstrating that sculptures reflect anatomical observation rather than stylized idealization.

CONCLUSIONS

Western classical sculpture proportions demonstrate a morphological envelope (43-50% upper pole) that encompasses contemporary aesthetic frameworks across 2,500 years. Rather than validating a single numerical target, these findings position the modern 45:55 ratio as a refined point within a broader historical range. This contextualization provides surgeons with historical perspective when counseling patients, while acknowledging that beauty standards reflect both biological and cultural influences requiring individualized surgical planning.
