Аннотация

OBJECTIVES

To examine the current performance of cancer screening programs in Italy, evaluate the evolving regulatory and clinical guideline framework, and discuss future opportunities for improving participation and equity through artificial intelligence.

STUDY

DESIGN

Narrative review and perspective analysis.

METHODS

A systematic analysis of Italian national screening data, existing legal frameworks, and recent national and European clinical guidelines was conducted. Emerging applications of artificial intelligence within these frameworks were evaluated.

RESULTS

Organized cancer screening in Italy is legally mandated under the Essential Levels of Care (LEA), yet actual population participation remains around 50% for breast and cervical screening, and lower for colorectal cancer. Significant geographic (North-South) and socioeconomic disparities persist. Recent frameworks, including the 2022 European Council recommendations, new European initiatives (ECIBC, ECICC, EC-CvC), and Italian guidelines (GISCi and GISMa-ISPRO), emphasize risk-based strategies and the regulated integration of artificial intelligence to optimize diagnostic accuracy and public health outreach.

CONCLUSIONS

While artificial intelligence offers powerful opportunities to enhance performance and equity in Italian cancer screening, its success depends on integration within unified public health strategies that align with national and European guidelines.
