BACKGROUND: Patients' beliefs and perceptions about medications might influence their adherence to treatment regimens, which is particularly crucial for chronic diseases like asthma. This study investigates the relationship between medication beliefs and adherence among asthmatic patients in Ethiopia.

METHODS: A cross-sectional study was conducted at Tikur Anbessa Specialized Hospital (TASH) in Addis Ababa, Ethiopia, from April 1 to May 31, 2023. Adult asthmatic patients on follow-up at chest clinic of the hospital were included in the study. The study involved 207 asthmatic patients who were assessed using the Medication Adherence Reporting Scale for asthma (MARS-A) and the Beliefs about Medicines Questionnaire (BMQ).

RESULTS: The mean adherence score was 3.76 ± 0.50, with 63.8% of patients demonstrating poor adherence. The average scores for specific necessity, specific concern, general overuse, and general harm were 13.55, 13.73, 9.45, and 13.66, respectively. Analysis of mean ranks using Mann Whitney U test revealed that there was no significant difference between adherent and non-adherent patients regarding beliefs on necessity of asthma medications and beliefs on general harm of medicines (p > 0.05). In contrast, the mean ranks for specific concern score [114.1 (n = 207, p < 0.001)] and general overuse score [121.0 (n = 207, p < 0.001)] were significantly higher in non-adherent patients. In multivariable linear regression, higher specific concern (B = -0.027, p = 0.003) and stronger beliefs in general overuse (B = -0.106, p < 0.001) were independently associated with lower MARS adherence scores.

CONCLUSION: The findings of the study revealed low level of adherence among asthmatic patients. Skepticism on the importance of asthma medication was demonstrated by low specific necessity score and low agreement rate to necessity statements. Higher specific concern and general overuse scores were associated with non-adherence. By incorporating structured patient education programs, motivational interviewing, and community health worker support into the care continuum, clinicians can directly counteract the identified belief barriers.
