Аннотация

PURPOSE: This prospective clinical case series evaluated the 5-year success of a bone anchorage technique using long trans-sinus implants for the rehabilitation of atrophic maxilla.

MATERIALS AND METHODS: Twenty patients were treated according to the all-on-four concept, receiving 45 standard implants and 35 long implants (18-24 mm) placed through the maxillary sinus. All patients were rehabilitated with metal-acrylic resin fixed complete prostheses. Prosthesis survival was defined as the presence of the prosthesis, with or without modifications. Follow-ups were conducted every 6 months, including hygiene instruction, prosthesis polishing, and clinical evaluation for suppuration, sinus pathology, peri-implantitis, gingival bleeding, pain, bleeding on probing, or implant mobility.

RESULTS: At the 5-year follow-up, clinical and radiographic examinations revealed no cases of sinus pathology or suppuration. All implants remained osseointegrated and functional, with a success rate of 100% (95% CI: 100-100%). Prosthesis survival was also 100% (95% CI: 100-100%), while prosthesis success was 48.1% (95% CI: 28.7-65.2%) after 5 years.

CONCLUSION: The use of long trans-sinus implants for immediate loading demonstrated predictable long-term outcomes, with excellent implant survival and high prosthesis survival.

CLINICAL RELEVANCE: This technique may provide clinicians with a reliable treatment option for rehabilitating atrophic maxillae, potentially reducing the need for grafting procedures while ensuring long-term functional stability.
