BACKGROUND: Systemic relapse after stage I melanoma is uncommon, but accounts for nearly one-third of metastatic melanoma cases. The prognostic impact of primary melanoma stage in advanced disease is unknown.

METHODS: Overall survival (OS) was analyzed in patients with advanced melanoma from the Spanish Melanoma Registry (2018-2024), stratified by primary stage.

RESULTS: Among 365 patients with metastatic melanoma treated at first line, 58 had prior stage I, 32 IIA, 80 IIB/C, and 195 III. Median time to relapse was 52 months for stage I, 30 months in IIA, 27 months in IIB/C and 25 months in stage III (p=0.0001). At systemic relapse, patients with previous stage I more frequently had ≥3 metastatic sites (OR 1.89, 95% CI 1.06-3.35), brain metastases (OR 4.02, 95% CI 2.13-7.58), and elevated LDH (OR 2.19, 95% CI 1.18-4.04). Median OS was shorter for patients with prior stage I (19.8 m, 95% CI 12, 55.9) versus stage IIA (60.1 m, 95% CI 52.8, NR), IIB/C (32.6 m, 95% CI 18.9, NR), and III (43 m, 95% CI 25.5, 62.3) (p=0.024).

CONCLUSIONS: Systemic relapse after stage I melanoma is associated with higher tumor burden and poorer survival than relapse after stage II/III.
