Аннотация

PURPOSE: To compare clinical and refractive outcomes of small-incision scleral suture fixation (SSF) versus intrascleral haptic fixation (ISHF) for secondary intraocular lens (IOL) implantation in adults.

SETTING

A single outpatient ophthalmology clinic.

DESIGN

Retrospective, single-center cohort study.

METHODS

Eyes undergoing small-incision SSF or ISHF between 2019 and 2024 were identified. Demographics, surgical indications, IOL types, refractive accuracy, and postoperative complications were analyzed. Refractive outcomes were evaluated using spherical equivalent refractive accuracy (SERA) and absolute SERA (ASERA).

RESULTS

A total of 132 eyes from 125 adult patients were included. Fifty-six eyes underwent SSF and 76 underwent ISHF. Indications included subluxated IOL (58%), dislocated IOL (17%), aphakia (20%), and uveitis-glaucoma-hyphema syndrome (5%). In the SSF group, MX60E or ET IOLs predominated, while CT Lucia 602 was most common in ISHF. Mean SERA was -0.45 D for SSF and -0.36 D for ISHF (p = 0.48); mean ASERA was 0.52 D and 0.54 D, respectively (p = 0.96). At postoperative month 3, 70% of SSF versus 62% of ISHF eyes were within ±0.50 D of target, and 85% versus 86% were within ±1.00 D. Complication rates were low and comparable, with trends toward higher early hypotony in SSF and more reoperations in ISHF.

CONCLUSIONS

Both small-incision SSF and ISHF are safe and effective for secondary IOL implantation in adults. Refractive outcomes were comparable, with SSF demonstrating slightly greater predictability and fewer reoperations, while ISHF was associated with less early hypotony. Technique selection should be guided by surgeon experience, IOL choice, and patient-specific factors.
