Аннотация

BACKGROUND: Massive hemoptysis in patients with complex pulmonary vascular disease is difficult to manage because surgery is frequently contraindicated, making endovascular therapy an important treatment option.

CASE SUMMARY: We report 3 patients with life-threatening hemoptysis caused by distinct rare etiologies: systemic-to-pulmonary collateral bleeding in Eisenmenger syndrome due to patent ductus arteriosus, complex collateral bleeding associated with left upper lobe aspergilloma, and post-tubercular collateral hemorrhage. Angiography-guided transcatheter embolization was individualized using particles, coils, and a vascular plug according to vascular anatomy. All patients achieved immediate hemostasis without major procedural complications, with sustained freedom from recurrent hemoptysis during 6-12 months of follow-up.

DISCUSSION: Successful bronchial artery embolization depends on meticulous angiographic evaluation and anatomy-driven device selection. This series demonstrates the versatility of endovascular techniques, including vascular plug implantation for high-flow bronchial collaterals in cases where conventional embolization is technically challenging.

TAKE-HOME MESSAGES: Individualized, anatomy-guided endovascular therapy provides a safe, effective, and organ-preserving treatment for life-threatening hemoptysis in complex pulmonary vascular disease. Careful procedural planning is essential for durable clinical success.
