AIM: Venous leg ulcers (VLUs) are the most common type of hard-to-heal lower extremity ulcers, significantly impacting quality of life and causing financial burden. The aim of the present study was to investigate the healing time and recurrence rate following polidocanol endovenous microfoam 1% injection in patients with hard-to-heal VLUs.

METHODS: Data from consecutive patients undergoing polidocanol endovenous microfoam injection between August 2019 and February 2021 at a university-affiliated referral wound care centre in Edison, NJ, US were retrospectively analysed. Their healing time and recurrence rate were compared with those of the general population of the wound care centre using one sample t-test, and p values <0.05 were considered statistically significant.

RESULTS: A total of 68 patients with hard-to-heal VLUs (Clinical, Etiological, Anatomical, and Pathophysiological (CEAP) classification 6) were treated with polidocanol endovenous microfoam 1% injection. No cases of deep vein thrombosis were diagnosed on the follow-up duplex scan one week post-injection. The 12-week healing rate was 100% and the average time to complete ulcer re-epithelialisation was 45±13 days (95% confidence interval: 41.2, 48.8 days). A statistically significant difference was found between the healing time of patients who were treated with polidocanol endovenous microfoam in addition to conventional therapy and that of the general population in the wound care centre, who had only conventional treatment (p<0.001). The recurrence rates of the polidocanol endovenous microfoam group at three months and at six months were 1/68 (1.5%) and 3/68 (4.4%), respectively, while the average recurrence rates of the general population in the wound care centre were 4.7% at three months and 9.5% at six months. There were no reported incidences of deep vein thrombosis or neurological symptoms associated with polidocanol endovenous microfoam 1% treatment.

CONCLUSION: In this study, using polidocanol endovenous microfoam 1% in patients with VLUs led to a shorter healing time and a lower recurrence rate.
