ABSTRACT: INTRODUCTION: This report describes our experience with PRESERFLO MicroShunt implantation with and without adjunctive subconjunctival Healaflow® as a modified bleb-forming filtration technique in glaucoma patients.
ABSTRACT: MATERIALS AND METHODS: A retrospective cohort analysis of 65 consecutive eyes undergoing single PRESERFLO MicroShunt implantation at a single center was performed. The Healaflow group (HG; n = 36, with Healaflow®) was operated between October 2024 and July 2025, and the control group (CG; n = 29, without Healaflow®) between October 2018 and December 2019. Medical records were reviewed for demographics, intraocular pressure (IOP), medication use and complications. IOP and topical medication were compared between groups preoperatively, on day 1, week 1, and at 1-3, 3-6, 6-12 and 12-18 months using unpaired t-tests and a two-way repeated-measures ANOVA.
ABSTRACT: RESULTS: Mean preoperative IOP was comparable between groups (24.4 ± 9.4 vs. 21.8 ± 8.2 mmHg; p = 0.24). IOP was significantly lower in the HG at 1-3 months (8.6 ± 4.0 vs. 14.0 ± 9.2; p = 0.007), 3-6 months (10.4 ± 5.4 vs. 14.9 ± 7.1; p = 0.031) and 6-12 months (11.0 ± 4.0 vs. 15.4 ± 5.3; p = 0.004), and remained nominally lower at 12-18 months (11.6 ± 6.0 vs. 13.8 ± 5.3; p = 0.254). The ANOVA showed a significant group-by-time interaction (p = 0.001) in favour of the HG. Topical medication was consistently lower in the HG (6-12 months: 0.41 vs. 1.38 compounds). Transient, self-limiting hypotony-related choroidal detachment/effusion occurred in 27.8% (HG) vs. 13.8% (CG); bleb revision was required in 8.3% vs. 13.8%.
ABSTRACT: DISCUSSION/CONCLUSION: PRESERFLO MicroShunt implantation with adjunctive subconjunctival Healaflow® may provide a greater and more sustained IOP reduction and a lower medication burden than MicroShunt with mitomycin C alone, at least in the early-to-mid postoperative period and in the absence of postoperative 5-fluorouracil. Transient hypotony was more frequent but reversible. Further, prospective and standardized comparisons are warranted.
