INTRODUCTION: Due to limitations in ablation energy delivery and myocardial wall thickness, endocardial ablation of atrial tachycardia can sometimes be challenging.
METHODS AND RESULTS: In this case, ablation of the left atrial posterior wall shifted the earliest endocardial breakthrough site from the posterior wall to the roof, suggesting a potential epicardial origin near left atrial roof. The atrial arrhythmia was successfully eliminated via a right pulmonary artery approach.
CONCLUSION: The right pulmonary artery approach may be an effective strategy for ablating epicardial atrial arrhythmias, particularly those originating from the left atrial posterior roof.
