Аннотация

Uncommon epidermal growth factor receptor (EGFR)-mutant lung adenocarcinoma comprises clinically and biologically heterogeneous subgroups with variable sensitivity to targeted therapy. Incomplete responses and eventual progression suggest that factors beyond receptor inhibition influence disease persistence during therapy. This hypothesis-generating narrative review examines whether apoptosis repressor with caspase recruitment domain (ARC), encoded by nucleolar protein 3 (NOL3), warrants investigation as a downstream node in the regulation of apoptosis within an aging-informed framework. The available evidence is considered across three levels: direct evidence from uncommon EGFR-mutant lung adenocarcinoma; evidence from broader EGFR-mutant lung adenocarcinoma or lung cancer; and evidence extrapolated from other cancers or nonmalignant tissues. Within the literature evaluated, no primary study directly demonstrated mutation-specific ARC/NOL3 enrichment, induction after EGFR inhibition, functional dependence, or an association with post-treatment tumor cell survival or clinical outcomes in uncommon EGFR-mutant lung adenocarcinoma. Accordingly, established ARC functions in caspase regulation, mitochondrial protection, and stress tolerance are not interpreted as evidence of disease-specific causality. Instead, they provide a biological rationale for testing whether uncommon EGFR signaling, regulation of apoptosis, and aging-associated stress responses intersect in selected tumor contexts. We outline the experimental and clinical evidence required to evaluate this hypothesis, including mutation-specific expression analyses, functional perturbation studies, longitudinal sampling, and biomarker validation. The framework identifies experimental and clinical priorities for evaluating the potential role of ARC/NOL3 in this disease setting.
