[A CASE OF CORPUS CAVERNOSUM ABSCESS CAUSED BY URETHRAL DILATION FOR URETHRAL STRICTURE].
Аннотация
A 67-year-old man presented to another clinic with a weak urinary stream. Urethral dilation was attempted, but insertion was difficult. Urethroscopy revealed a penile urethral stricture with a false passage. He later developed urinary retention and, after suprapubic cystostomy, was referred to us. Retrograde urethrography demonstrated an approximately 4-cm penile stricture with contrast extravasation into the corpora cavernosa, and contrast-enhanced MRI revealed a corpus cavernosum abscess measuring approximately 33 mm in maximal diameter. Because simultaneous abscess drainage, fistula repair, and urethroplasty were considered impractical, we performed perineal urethrostomy. Three months later, MRI showed a residual abscess cavity; however, in the absence of pain or signs of infection, we deferred further procedures such as fistula repair or abscess drainage and continued observation. Urethral dilation is not indicated for penile strictures, and indiscriminate use can cause serious complications, including corpus cavernosum abscess.
