Аннотация

Around-the-clock (ATC) opioid dosing has anchored cancer pain management since an era of advanced disease, continuous pain, and short survival, when undertreatment was the dominant clinical failure. Contemporary oncology increasingly involves prolonged survival and chronic treatment-related pain, and advances in behavioral pharmacology and addiction neuroscience challenge the assumption that dosing schedules are behaviorally neutral. In non-addicted individuals, opioid self-administration tracks the presence of pain, consistent with negative reinforcement; with chronic use this coupling weakens, and opioid-taking may become progressively pain-independent. Integrating experimental (Comer, Colpaert), neurobiology (Volkow, Koob), and human clinical data (Von Korff, Edler-Buggy), this comment proposes that preservation of pain-contingent administration may help distinguish therapeutic use from problematic use. The framework does not oppose ATC dosing for continuous severe cancer pain; but-recognizing that pain-contingency is difficult to measure directly and that relevant neuroadaptation emerges with regular use itself-it offers a candidate, largely guideline-concordant principle for opioid stewardship in survivorship: keeping opioid exposure linked to meaningful symptom relief whenever clinically feasible.
