Аннотация

Ventriculoperitoneal shunt (VPS)-associated bacterial meningitis is a significant neurosurgical complication that can occasionally present atypical symptoms. We report a rare case of Streptococcus pyogenes meningitis in a 40-day-old infant with a history of hydrocephalus managed by VPS placement. The patient presented with vomiting, diarrhea, fever, and -abdominal distension. Microbial culture of the removed VPS valve confirmed the diagnosis, revealing Streptococcus pyogenes. Despite the strain's sensitivity to penicillin G, the patient was treated with cefotaxime and vancomycin due to negative cerebrospinal fluid (CSF) cultures and clinical improvement under this regimen. Early removal of the VPS and initiation of targeted antibiotic therapy, along with the placement of an external ventricular drain (EVD), led to a full recovery. This case highlights the importance of prompt diagnosis, tailored treatment strategies, and the critical role of infection control measures in managing VPS-related complications.
