Аннотация

Periodontal disease is a chronic inflammatory condition driven by a dysregulated host immune response to microbial biofilm, resulting in connective tissue breakdown and alveolar bone loss. Host modulation therapy (HMT) aims to reduce host immune-inflammatory response and promote periodontal stability rather than directly targeting bacteria. Currently available host modulating agents include sub-antimicrobial dose doxycycline, which inhibits matrix metalloproteinases and collagen breakdown, as well as nonsteroidal anti-inflammatory drugs and bisphosphonates, which modulate inflammatory pathways and bone metabolism, respectively. Adjunctive approaches such as statins, omega-3 fatty acids, and certain vitamins have also demonstrated anti-inflammatory and bone-protective effects. Emerging therapies continue to explore targeted cytokine modulation and novel pathways involved in host response. Clinically, the use of HMT is indicated in patients with persistent inflammation, high-risk disease progression, or systemic conditions that influence periodontal breakdown; its use requires careful case selection and individualized decision-making. Limitations, however, such as variable efficacy, potential adverse effects, patient compliance, and concerns regarding long-term use, must be considered when integrating host modulation into periodontal therapy.
