[Research progress on mechanism of traditional Chinese medicine in treating acute lung injury based on endoplasmic reticulum stress].

Acute lung injury(ALI) is a severe pulmonary condition with a high mortality rate, and is also a leading cause of morbidity and mortality among intensive care unit patients. Endoplasmic reticulum stress(ERS) is a key central mechanism in ALI. It primarily regulates endoplasmic reticulum function by activating the unfolded protein response and modulating three core pathways-inositol-requiring enzyme 1(IRE1), activating transcription factor 6(ATF6), and protein kinase R-like endoplasmic reticulum kinase(PERK)-which in turn regulate processes such as oxidative stress, ferroptosis, and autophagy. It is involved in processes including apoptosis, inflammatory storm, and calcium homeostasis, thereby influencing the progression of ALI. TCM offers therapeutic advantages through its multi-targeted and multi-pathway approach, improving the course of ALI by modulating ERS. This paper reviewed the experimental evidence and mechanisms for single herbs and their chemical constituents or extracts(casticin, ursolic acid, toosendanin, resveratrol, ginsenosides, and total flavonoids from Patrinia villosa), TCM compound formulas(Zhenwu Decoction, Buzhong Yiqi Decoction, and Dushen Decoction), and Chinese patent medicines(Lianhua Qingwen Capsules and Jingfang Granules) to modulate ERS for the treatment of ALI. It elucidated the scientific rationale behind the use of TCM to modulate ERS for ALI treatment, providing a mechanistic basis and research directions for the prevention and treatment of ALI by integrated traditional Chinese and western medicine.
