OBJECTIVES: To investigate the correlation between tissue stiffness, as measured across various target tissues using multifrequency three-dimensional magnetic resonance elastography (3D-MRE), and tumor responses to transarterial chemoembolization (TACE), as well as post-treatment prognosis in patients with hepatocellular carcinoma (HCC).
METHODS: HCC patients scheduled for TACE treatment were prospectively recruited and received multifrequency 3D-MRE before the initiation of TACE procedure. Measurements of tumor stiffness (TS), liver stiffness (LS), and spleen stiffness (SS) were conducted at frequencies of 60 Hz and 30 Hz, respectively. Treatment response was assessed using the mRECIST. Patient follow-up was conducted to evaluate overall survival outcomes.
RESULTS: The final patient cohort consisted of 43 patients (mean age: 56.66±10.13 years; 34 males), including 16 non-responders and 27 responders. Non-responders exhibited significantly elevated TS (4.10±2.54 vs. 2.68±1.61kPa, p=0.035), LS (2.88±1.43 vs. 1.97±0.59kPa, p=0.026), and SS (4.03±0.90 vs. 3.13±1.08kPa, p=0.008) at the frequency of 30Hz. Firth's penalized univariable logistic analysis identified age (adjusted odds ratio [OR]=0.93, P=0.036), tumor number (OR=0.69, P=0.035), LS at 60Hz (OR=0.79, p=0.046), TS at 30Hz (OR=0.73, p=0.037), LS at 30Hz (OR=0.44, p=0.008), and SS at 30Hz (OR=0.44, P=0.009) as potential predictors of treatment response. The area under the receiver operating characteristic curve for predicting treatment response was 0.69 for age, tumor number and TS at 30Hz, 0.67 for LS at both 60Hz and 30Hz, and 0.75 for SS at 30Hz. Univariable Cox regression identified TS at both 60Hz (hazard ratio=1.15, p=0.014) and 30Hz (hazard ratio=1.40, p=0.010) as potential predictors of overall survival.
CONCLUSION: Spleen stiffness measured at 30 Hz via 3D-MRE may serve as a potential predictor of treatment response to TACE in HCC patients, whereas tumor stiffness measured at both 60Hz and 30Hz appears to correlate with overall survival. Further research is necessary to validate these preliminary associations.
