This case highlights an uncommon but clinically important mechanism of atrial fibrillation recurrence after Pulsed field ablation. It emphasizes the importance of ablation along the ridge during the first session and also the need for careful evaluation of epicardial conduction, particularly in patients with a left common pulmonary vein.
