BACKGROUND: Adolescent obesity remains a major public health challenge in the United Kingdom among those aged 10 to 19 years, with lasting consequences for health and wellbeing. Using the Commercial Determinants of Health framework, this perspective analyses how commercial dynamics interact with regulatory gaps and delayed policy implementation to sustain obesogenic environments for adolescents. Drawing on peer reviewed studies, surveillance data and international evidence, the analysis focuses on levels 3 and 4 of the framework, namely sectoral governance and the commercial configuration of food environments, to clarify how policy design and commercial adaptation shape adolescent exposure pathways. The paper argues that fragmented governance and regulatory pacing enable commercial strategies shaping food environments, including marketing, pricing and product placement, to persist despite existing policy measures such as the Soft Drinks Industry Levy and restrictions on products high in fat, sugar and salt. Limitations in national surveillance, particularly the under-representation of older adolescents within the National Child Measurement Programme, further constrain targeted policy responses.
CONCLUSION: Evidence from the Young People at a Healthy Weight initiative illustrates how coordinated governance and community systems approaches can reshape local environments and reduce inequalities in adolescent obesity. By linking governance fragmentation with commercial adaptation, this perspective provides a structural explanation for the limited impact of isolated policy interventions and highlights the need for coordinated multilevel responses.
