AIM: Honeymoon phase (HP) of type 1 diabetes (T1D) represents a transient period of partial remission with preserved β-cell function. This study assessed the prevalence, duration, and early predictors of HP in newly diagnosed Asian-Indian T1D subjects.
METHODS: In this longitudinal cohort study, 76 individuals aged 12-25 years were recruited within two months of T1D diagnosis. HbA1c, insulin dose-adjusted A1c (IDAA1c), fasting and mixed-meal stimulated C-peptide were assessed every three months till 18 months.
RESULTS: 42% (25) of subjects entered HP defined as IDAA1c ≤ 9, with median onset of 6 months and median duration of 9 months. Most baseline parameters were comparable, although baseline total daily insulin dose/kg body weight (TDD) and IDAA1c were significantly lower in HP group. Subjects entering HP demonstrated significantly lower TDD, superior glycemic control and preserved C-peptide levels. At 3 months, stimulated C-peptide > 2.10 ng/mL showed strongest predictive performance for HP occurrence, AUC 0.89, followed by HbA1c < 9.75%, AUC 0.83, TDD < 0.58 IU/kg/day, AUC 0.79 and Fasting C-peptide > 1.10 ng/mL, AUC 0.77.
CONCLUSIONS: 3-month C-peptide, HbA1c, and TDD may serve as early predictors to identify patients likely to enter HP and may support selection of candidates for β-cell preserving interventions.
