Аннотация

BACKGROUND

Pediatric alpha-gal syndrome remains underrecognized because delayed reactions disconnect symptoms from mammalian exposure and gastrointestinal-only presentations may bypass allergy evaluation. Overly broad avoidance may subsequently impose nutritional and psychosocial burden during growth.

OBJECTIVE

To synthesize pediatric evidence and provide a directed management framework emphasizing diagnostic delay, least-restrictive avoidance, nutritional burden, natural history, and reassessment.

DATA SOURCES: A literature search was conducted using PubMed/MEDLINE for articles published through August 2026.

STUDY SELECTIONS: Pediatric and age-stratified primary studies were prioritized. Adult and mixed-age studies informed areas with limited pediatric evidence, with consideration of their applicability to children.

METHODS

Evidence was qualitatively synthesized by source; no risk-of-bias assessment was performed.

RESULTS

Pediatric cohorts document prolonged diagnostic delay and frequent gastrointestinal-predominant presentations. Tick exposure is a key etiologic and diagnostic clue, but lack of a recalled bite does not exclude alpha-gal syndrome. Diagnosis requires a compatible delayed phenotype plus detectable alpha-gal-specific IgE; sensitization alone does not establish alpha-gal syndrome. Mammalian meat and internal organ meat are the core exclusions, whereas tolerated dairy should be preserved and additional restrictions individualized. Potential mammalian origin of a medication or vaccine warrants product-specific assessment rather than blanket avoidance. No prospective pediatric study has quantified dietary intake, nutrient status, growth, bone health, or quality of life. A 2026 meta-analysis of 1,162 predominantly adult or mixed-age patients found symptomatic improvement in 86.5% but complete resolution in 42.7%; no pediatric remission rate or validated challenge/reintroduction protocol exists.

CONCLUSION

Care should combine cross-specialty evaluation, phenotype-directed testing, least-restrictive avoidance, nutritional surveillance, tick-bite prevention, emergency preparedness, and planned reassessment toward the broadest safely tolerated diet.
