Аннотация

PURPOSE: To compare the mechanical behavior, intraoperative events, and postoperative outcomes of two three-piece intraocular lenses (IOLs) with different haptic materials-polymethyl methacrylate (PMMA) and polyvinylidene fluoride (PVDF)-implanted using the Yamane fixation technique.

METHODS

This retrospective comparative study included eyes undergoing Yamane scleral fixation with either a PMMA-haptic IOL (Alcon MA60AC) or a PVDF-haptic IOL (Eyecryl Plus TP613). Preoperative and postoperative month 6 best-corrected visual acuity (BCVA), intraocular pressure (IOP), and cylindrical refraction, as well as intraoperative haptic-related complications including haptic tip breakage and optic-haptic junction separation, postoperative IOL dislocation, and other postoperative complications were recorded and analyzed.

RESULTS

Seventy-four eyes (37 per group) were included. Baseline demographic and preoperative clinical characteristics were similar between groups. At 6 months, BCVA improved significantly in both groups without a significant intergroup difference, and postoperative IOP values and cylindrical refraction were comparable. Intraoperative complications were more frequent in the Alcon group (32.4%, 95% CI 19.6-48.5% vs. 5.4%, 95% CI 1.5-17.7%; p = 0.003), with haptic-related complications occurring exclusively in this group. Overall postoperative complication rates were similar; however, IOL dislocation occurred only in the Alcon group (16.2%, 95% CI 7.7-31.1% vs. 0%; p = 0.025). Other postoperative complications did not differ significantly.

CONCLUSION

Both IOLs yielded comparable improvements in visual acuity; however, all haptic-related complications and IOL dislocations occurred exclusively in the Alcon MA60AC group. These findings suggest that IOL selection is an important consideration in Yamane fixation, and the Eyecryl Plus TP613 with its PVDF haptics and more favorable design profile may offer greater biomechanical reliability.
