BACKGROUND/OBJECTIVES: Although the use of zero-sugar products containing non-nutritive sweeteners (NNSs) has increased, limited information is available on the current NNS use across product categories and its relationships with consumer perceptions and dietary quality. This study examined the consumption patterns and factors associated with current NNS use.

SUBJECTS/METHODS: Four hundred and sixteen adults residing in the Chungcheong region of Korea participated in an online survey between May and June 2024. The participants were classified as current or non-current NNS users based on their consumption of zero-sugar products containing NNSs. The questionnaire assessed the general characteristics, consumption patterns, awareness and perceptions, information needs, sugar intake behaviors, sugar reduction awareness, and the Nutrition Quotient (NQ). The group differences were examined using χ2 tests, independent t-tests, and analysis of covariance. Multivariable logistic regression analysis was conducted to identify the factors associated with current NNS use.

RESULTS: Overall, 218 participants (52.4%) were current NNS users. Carbonated beverages were the most frequently consumed product category, followed by tabletop sweeteners, coffee products, and sports drinks. The current users had greater awareness and more favorable safety perceptions than non-current users. In the fully adjusted model, adults aged 30-39 yrs had higher odds of current NNS use than those aged 19-29 yrs (odds ratio [OR], 2.55; 95% confidence interval [CI], 1.44-4.49). Higher sugar reduction awareness was associated with higher odds of current use (OR, 1.64; 95% CI, 1.17-2.29), whereas each 10-point increase in the total NQ score was associated with lower odds of current use (OR, 0.79; 95% CI, 0.63-0.99).

CONCLUSION: Current NNS use was associated with stronger sugar reduction awareness but not with better overall dietary quality, highlighting the need for practical guidance on NNS safety and balanced dietary practices.
