Decreases in Cancer Mortality During the COVID-19 Pandemic and Mortality Displacement in Aged Societies.

Objectives. To examine whether pandemic-era changes in cancer mortality reflect true improvements or mortality displacement and whether patterns differ by population aging, with updated Global Burden of Disease (GBD) mortality estimates from the Institute for Health Metrics and Evaluation. Methods. We conducted a cross-national ecological analysis using GBD 2023 data from 204 countries and territories. Countries were classified as aged if more than 14% of the population was aged 65 years or older in 2019. The exposure was cumulative COVID-19 age-standardized mortality rate (ASMR), and the outcome was change in all-cancer ASMR. Results. Among aged societies (n = 55), higher COVID-19 mortality was correlated with larger decreases in cancer mortality (r = -0.45; P = .001). No correlation was observed in non‒aged societies (n = 149; r = 0.08; P = .34), and correlations differed (P < .01). Across all countries and territories, the overall correlation was weak (r = 0.13; P = .06), indicating the inverse correlation was confined to aged populations. Public Health Implications. Decreases in cancer mortality during the COVID-19 pandemic may reflect mortality displacement rather than true improvements, particularly in aging populations. (Am J Public Health.
