PURPOSE: To introduce a novel ILM peeling technique for S2 and S3 retinoschisis, termed 'Personalised ILM peeling', which selectively peels the ILM only within the schisis area while minimising postoperative complications.
METHODS: 10 eyes from 9 patients with MRS underwent 'Personalised ILM peeling'. Preoperatively, the OCT radial scans, 3D Wide scans and retinal thickness map were comprehensively evaluated to delineate the extent of the schisis area and determine the specific region requiring ILM peel. Pre- and postoperative visual acuity, retinal thickness, including retinoschisis height, and visual field were analyzed.
RESULTS: No case of recurrence were observed during a follow-up period exceeding 24 months. No Postoperative complications occurred in any of the 10 cases. Compared with preoperative measurements, postoperative visual acuity and retinal thickness, including retinoschisis height, demonstrated significant improvement.
CONCLUSIONS: Personalised ILM peeling is proposed as a novel, minimally invasive surgical approach for MRS. Many cases with high myopia are accompanied by visual field defects and it is expected that this will avoid damage to the fragile retina and nerve fibres.
