BACKGROUND: The patients of lung cancer combined with interstitial lung disease (LC-ILD) account for a high proportion, and their prognosis is poor. Currently, no standard treatment regimen had been established for this population. This study aimed to summarize the clinical characteristics and treatment outcomes of patients with LC-ILD, and to explore factors associated with marked ILD progression under different tumor treatment strategies.
METHODS: The clinical characteristics, tumor treatment, and follow-up data of 49 LC-ILD patients hospitalized in the Department of Pulmonary and Critical Care Medicine at Peking Union Medical College Hospital from January 2018 to December 2024 were collected. The efficacy and follow-up outcomes of different treatment modalities were observed; the survival analysis was performed, and the potential influencing factors for the significant progression of ILD were analyzed.
RESULTS: LC-ILD is most commonly of the usual interstitial pneumonia (UIP) pattern, with tumors frequently located in fibrotic regions. The median progression-free survival (PFS) was 7.0 months for advanced non-small cell lung cancer (NSCLC) and 4.0 months for small cell lung cancer (SCLC). Of the 36 patients with completed follow-up and treatment, 10 showed radiographic progression of interstitial lesions, with 4 deaths occurring in this subgroup. Logistic regression analysis indicated that radiotherapy was significantly correlated radiological progression of ILD (P<0.05).
CONCLUSIONS: For patients with LC-ILD, individualized evaluation including chest high resolution computed tomography and pulmonary function test (particularly diffusing capacity) should be performed before initiating therapy. The therapeutic approach should balance efficacy and safety, and special attention should be given to the risk of radiological progression of ILD associated with radiotherapy.
