Redefining Cholangiocarcinoma Management: Breakthroughs and Their Clinical Implications for Clinical Practice.
Аннотация
Cholangiocarcinoma (CCA) is a heterogeneous group of malignancies that originates in the biliary epithelium and is anatomically classified into intrahepatic (iCCA) and extrahepatic (eCCA) subtypes. These tumors often progress silently and are associated with a poor prognosis, limiting the opportunities for timely intervention. Most patients do not have identifiable risk factors. Recent improvements in imaging techniques and next-generation sequencing (NGS) have enhanced diagnostic accuracy and enabled identification of actionable molecular alterations (e.g., FGFR2 fusions, IDH1 mutations). These advances have supported biomarker-driven therapies and integration of immunotherapy into first-line treatment. However, persistent gaps in risk stratification and early detection remain major barriers to improving outcomes. This review highlights recent advancements in CCA treatment and management, emphasizing practical considerations for gastroenterologists and hepatologists.
