Cancer metastasis: biological mechanisms, therapeutic bottlenecks, and emerging interventions.
Аннотация
Metastatic cancer remains a major clinical challenge, which is driven by the continuous adaptation of disseminated tumor cells to diverse tissue environments and therapeutic pressures. Although current treatments, including conventional therapies, small-molecule drugs, antibody-drug conjugates, and immunotherapies, have improved clinical outcomes in selected patients, they rarely achieve durable disease control, largely because each stage of the metastatic cascade presents distinct therapeutic bottlenecks. This review examines these bottlenecks and identifies therapeutic vulnerabilities that can be targeted at each metastatic stage. Cross-stage adaptive programs, encompassing genetic, epigenetic, metabolic, immune, and microenvironmental alterations, are linked to the specific modes of therapeutic failure they drive, thereby connecting underlying mechanisms with their corresponding clinical consequences. Emerging therapeutic strategies are discussed in the context of the metastatic stage, and general design principles for combination regimens are proposed. We also highlight the potential of liquid biopsy-guided dynamic treatment adjustment and advanced preclinical models to inform patient selection, treatment timing, and the design of therapies aimed at overcoming resistance. By integrating metastatic biology, mechanisms of therapeutic failure, and emerging interventions, this review provides a framework for understanding and targeting the bottlenecks that limit durable disease control in metastatic cancer.
