Аннотация

Penile fracture is caused by rupture of the tunica albuginea and typically presents with acute pain, a snapping sound, and rapid detumescence. False penile fracture mimics these clinical features but occurs without tunical rupture, most commonly due to dorsal venous injury. Distinguishing between these two conditions is often challenging, particularly in emergency settings where imaging modalities may not be readily available. Current guidelines recommend surgical exploration when the diagnosis is uncertain. A man in his 50s presented with penile swelling during sexual intercourse. Progressive enlargement of a hematoma was observed, and a true penile fracture could not be excluded. Surgical exploration revealed no tunical rupture but identified an injury to the superficial dorsal vein, leading to a diagnosis of false penile fracture. Hemostasis was achieved surgically, and the postoperative recovery was uneventful, with preservation of erectile function. False penile fracture is clinically difficult to distinguish from true penile fracture. Surgical exploration remains appropriate in cases of diagnostic uncertainty. This case represents a rare report from Japan and may contribute to the establishment of standardized Japanese terminology for false penile fracture. It may also enhance clinical awareness of this condition and facilitate its differential diagnosis in Japan.
