Background: Resilience in high-stress occupations is commonly framed as a trait, an outcome, or the absence of psychopathology. None of these framings captures what practitioners in emergency medical services actually do, and interventions built on trait-based models have produced weak or inconsistent effects. Objective: This study argues that frontline resilience in emergency medical services is better understood as a domain-specific expert skill, and that work mode - a deliberately entered state, akin to an athlete's game face, in which the demands of the task rather than the emotional force of the scene govern functioning - is its central empirical signature. The aims were to identify factors that intensify traumatic impact, characterise the strategies practitioners use to maintain functioning, and explore systemic and relational barriers that constrain coping.Method: Twenty Queensland Ambulance Service paramedics and dispatchers (tenure 2-38 years) participated in semi-structured interviews, analysed via reflexive thematic analysis. Results: Three intensifiers of traumatic impact (unpredictability, moral weight, personal threat) defined the operational demands against which resilience was enacted. Three coping themes (baseline buffers, work mode, social scaffolding) described the architecture of sustained functioning. Work mode exhibited the defining features of skilled performance: domain specificity, a phased temporal structure (preparation, containment, recovery), and a developmental trajectory from effortful to automatic execution. Two further themes (organisational distrust, the outsider gap) described the systemic and relational conditions that constrain coping. Conclusions: Treating resilience as expertise shifts the field from descriptive stressor-outcome accounts towards assessable, trainable capacities, and exposes the limitations of trait-based self-report measures that cannot distinguish genuine resilience from unsustainable stoicism.
