Memory-Based Learning Disabilities: A Syndrome without a Pigeonhole-Towards a Mechanism-First Assessment Framework.
Аннотация
Memory-based learning disabilities represent a distinct and underrecognized clinical entity that current diagnostic classification systems fail to capture. This theoretical synthesis argues that these difficulties constitute an identifiable syndrome defined by neuropsychological vulnerabilities in declarative memory systems, particularly those dependent on hippocampal function. Evidence from cognitive neuroscience, developmental neuropsychology, and educational psychology converges to identify a distinctive phenotypic profile that differentiates memory-based learning disabilities from other developmental learning disorders. Five etiologic pathways are identified, including hypoxic ischemic, toxic metabolic, epileptic electrical, genetic congenital, and perinatal traumatic pathways, which converge on hippocampal and related medial temporal networks to produce an identifiable phenotype. Classification systems organized by academic domain systematically overlook children whose primary difficulty lies in memory-based learning rather than phonological processing, attention, or general intellectual ability. Comprehensive assessment of declarative memory function may improve the identification and characterization of children whose learning difficulties are not adequately captured by current academic domain based classifications. Memory-based learning disabilities should be recognized as a distinct syndrome, with important implications for assessment, intervention, and future research.
