BACKGROUND: The deployment of defective airbag inflators can generate high-velocity metallic fragments that cause face, neck or chest penetrating injuries with potentially lethal consequences. This study aimed to describe the injury patterns, management, and outcomes of suspected airbag-related penetrating injuries (ARPI) among patients in a Middle Eastern country.

METHODS: A retrospective observational study of patients presenting with penetrating injuries suspected of being related to defective airbag deployment. Data were obtained from the Qatar National Trauma Registry (QNTR), electronic medical records, emergency medical services (EMS), emergency department (ED), and Mortuary. Data were collected and analyzed descriptively.

RESULTS: A total of 16 patients had suspected ARPIs between April 2019 and February 2026. The mean age was 33.1 ± 8.3 years, and 93.8% were male. Metallic foreign bodies (FBs) from airbag inflator fragmentation were documented in 11/16 (68.8%) patients by imaging, and surgical removal while in 4/16 patients there were only entry-exit wounds after deployment of airbags following car crash. In one case, airbag deployed while mechanic was repairing a car steering wheel. Penetrating neck (43.8%), thoracic (31.3%), and maxillofacial injuries (31.3%), predominantly mandibular fractures, were most common. Surgical intervention was required in 11 (68.8%) patients and 7/16 required intensive care unit admissions. Five patients died (31.3%), including one at the scene (transported directly by the EMS, no CPR, and moved to the hospital mortuary), two died at the ED after unsuccessful CPR and two died during the hospital course (one presented with cardiac arrest and died on the sixth day and the other one required prolonged ECMO for acute respiratory distress syndrome (ARDS) and died after 4 months).

CONCLUSIONS: Airbag inflator rupture can produce serious injury patterns resembling patterns of ballistic trauma, characterized by penetrating neck wounds, maxillofacial fractures and thoracic injuries. These injuries are potentially associated with high mortality, surgical intervention, and critical care utilization. The full extent of these injuries is undetermined as some victims may have simply died at the scene without the benefit of trauma center evaluation or post-mortem examination.
