INTRODUCTION: Global demographic dynamics is shaping a steady «silver shift.» The growing proportion of the elderly population and the increase in the number of elective surgical interventions in patients of older age groups pose a challenge to the healthcare system. Against this background, the problem of preoperative preparation of geriatric patients is of strategic importance. Patients with signs of senile asthenia need multidisciplinary preoperative training to reduce the frequency of postoperative complications and improve their quality of life. The aim of the study is to substantiate the need for systematic preoperative preparation of geriatric patients with senile asthenia syndrome.
MATERIALS AND METHODS: A comparative analysis of two groups was carried out: Group 1 (30 patients, retrospective traditional management) and Group 2 (30 patients, prospective management according to the geriatric protocol). Training in the study group began 4 weeks before surgery, and included screening for asthenia (the «Age is not a hindrance» questionnaire), a comprehensive geriatric assessment, additional oral nutrition at risk of body weight deficiency, and correction of drug therapy.
RESULTS: The systematic use of nutritional support and drug correction of anemia provided a significant reduction in the frequency of infectious complications, accelerated tissue healing and reduced the severity of pain. Normalization of hemoglobin reduced the need for blood transfusions, improvement of protein metabolism contributed to a reduction in the duration of hospitalization and the risk of repeated visits.
CONCLUSION: The integration of nutritional support and correction of anemia into the preoperative preparation of geriatric patients with senile asthenia syndrome is a reasonable and clinically effective direction.
