BACKGROUND AND

OBJECTIVES

Oncoplastic breast surgery integrates oncologic resection with reconstructive techniques to optimize both tumor control and aesthetic outcomes. The Modified Oncoplastic Lift and Lymphatic Excision and Reconstruction (MOLLER) technique is a reconstructive approach incorporating lymphatic preservation principles while aiming to preserve breast contour. However, perioperative outcomes remain incompletely characterized.

METHODS

A single-center retrospective cohort study included adult women undergoing partial mastectomy or lumpectomy for malignant or benign breast conditions with the MOLLER technique (2011-2021). Primary outcomes were immediate and delayed postoperative complications; secondary outcomes were re-excision and radiation-related complications.

RESULTS

A total of 136 patients were included. Median follow-up was 3 years. Immediate complications were infrequent: hematoma and wound dehiscence each occurred in 3 patients (2.2%); seroma, infection, and paresthesia each in 2 (1.5%). Delayed complications were uncommon: arm lymphedema in 5 patients (3.7%); fat necrosis and breast lymphedema each in 4 (2.9%). The re-excision rate was 4.4%. Radiation-related complications were uncommon, most commonly skin discoloration (7.4%) and skin thickening (5.1%); no delayed wound healing occurred.

CONCLUSIONS

The MOLLER technique is a safe reconstructive approach associated with low rates of postoperative complications, re-excision, and lymphedema. Its lymphatic preservation principles may be associated with reduced lymphatic morbidity. Prospective studies are warranted to validate these findings.
