Аннотация
Robot-assisted radical prostatectomy (RARP) is a standard treatment for localized prostate cancer (PCa). As new, lower-cost robotic platforms reach Latin America, comparative evidence remains scarce. We compared complications, and short-term oncological and functional outcomes between the da Vinci and the new Toumai platforms in RARP. Single-center, prospective, non-randomized study of patients older than 40 years with localized PCa undergoing RARP between June 2025 and July 2026. The first 100 consecutive Toumai cases were compared with 100 consecutive da Vinci cases operated during the same period; platform allocation depended on operating-room availability, without matching or randomization. Salvage RARP was excluded. Perioperative variables, Clavien-Dindo complications, pathological outcomes, positive surgical margins (PSM), undetectable PSA (< 0.1 ng/mL) and continence (pads/day) at one and three months were analyzed. Two hundred patients were analyzed (100 Toumai vs. 100 da Vinci) with comparable baseline characteristics. Operative time (215.0 vs. 200.3 min) and console time (193.6 vs. 178.0 min) were longer with Toumai (p = 0.001 and p < 0.001, respectively), while docking time did not differ significantly (21.4 vs. 22.3 min; p = 0.147). Hospital stay (1.6 days; p = 0.819), catheter duration (10.7 vs. 10.8 days; p = 0.671), overall complications (18% vs. 10%; p = 0.103), PSM (19% vs. 17%; p = 0.713) and undetectable PSA (96% vs. 93%; p = 0.352) were similar. Continence (no pads) was 84% vs. 73% at one month (p = 0.058) and 87% vs. 85% at three months (p = 0.314). In a high-volume center, the da Vinci and Toumai platforms showed comparable short-term oncological, functional, and complication outcomes, including similar platform docking times despite longer total operative and console times with Toumai.
