BACKGROUND: Subjective cognitive decline (SCD) is defined as a self-perceived reduction in cognitive function and is associated with future cognitive impairment. However, few clinical trials have explored the impact of nonpharmacological interventions in older adults with SCD. Time-restricted eating (TRE) restricts food intake to a daily eating window and has shown improvements in cardiometabolic markers associated with cognitive impairment.
OBJECTIVE: The Fasting to provide Energy Needed to Help Adults in Need of Cognitive Enhancement (Fasting ENHANCE) study is a rater-blinded, parallel-group, randomized controlled trial, designed to evaluate the effects of TRE on cognitive function in overweight older adults with SCD.
METHODS: The study aims to enroll older adults (aged ≥65 years) who are overweight (BMI ≥25 kg/m2) and have SCD without objective impairment. Participants are randomized to either a 6-month TRE or Successful Aging (SA) group. The TRE group is instructed to restrict eating to an 8-h window, and the SA group to maintain current lifestyle behaviors. The primary outcome is change in cognitive function, assessed using the Repeatable Battery for the Assessment of Neuropsychological Status and Montreal Cognitive Assessment at baseline, 3 months, and 6 months. Secondary outcomes include physical function, blood biomarkers, and self-reported mood, sleep, and quality of life.
CONCLUSION: The Fasting ENHANCE study protocol outlines the methodology for evaluating the effect of TRE on cognitive and metabolic health in older adults with SCD. Results will inform future nonpharmacological interventions aimed at attenuating cognitive decline.
