PURPOSE: To investigate survival outcomes in frail, older patients with colorectal cancer who were deemed non-operable despite technically resectable tumors, compared to propensity score matched patients who underwent curative-intent surgery.

METHODS: This was a propensity score-matched retrospective cohort study. Overall survival was estimated using the Kaplan-Meier method. A univariate Cox proportional hazards model was applied to estimate hazard ratios (HRs) and their corresponding 95% confidence intervals (CIs).

RESULTS: From a total of 1667 patients, assessed at the Department of Surgery, Zealand University Hospital, Denmark, between January 1, 2020, and April 30, 2024, 67 were deemed non-operable and 805 patients underwent elective, curative intent surgery. After applying PSM, each study cohort included 36 patients. Both, before and after propensity score matching, surgery was significantly associated with better survival, HR: 11.73, 95% CI: 8.48-16.21, p < 0.01 and HR: 5.06, 95% CI: 2.57-9.95, p < 0.001, respectively. The results of the sensitivity analysis were consistent.

CONCLUSION: In the propensity score-matched cohort, curative intent surgery was significantly associated with longer overall survival compared with patients who were deemed non-operable.
