Designing for success in early phase haemophilia clinical trials: historical lessons and emerging principles for next generation therapeutics.
Аннотация
INTRODUCTION
Hemophilia drug development has progressed through five therapeutic eras: plasma-derived concentrates, recombinant proteins, extended-half-life molecules, non-factor rebalancing agents, and gene therapy. Each transition depended not only on molecular innovation but also on early-phase trials that addressed critical translational and safety questions before pivotal testing.
AREAS COVERED: This review examines the evolution of early-phase hemophilia trial design across these five eras, identifies lessons from successful and unsuccessful programs, and proposes a ten-domain framework encompassing translational science, population selection, dose-finding, endpoints, biomarkers, laboratory standardization, immunogenicity, safety, statistics, and regulatory strategy. The framework is applied to genome editing, RNA therapeutics, rebalancing agents, and synthetic coagulation proteins, drawing on published evidence through mid-2026 and the author's experience as a clinical trial investigator.
EXPERT OPINION: Therapeutic success requires both molecular innovation and rigorous early-phase trial design. An ingenious product cannot compensate for failure to define its therapeutic window, clinically meaningful benefit, or implementation requirements. Experience with non-factor and gene therapies shows that mechanisms with compelling preclinical rationale may fail, cause harm, or remain clinically underused when durability, immunogenicity, and real-world implementation are treated as late-stage considerations. Addressing these issues from first-in-human studies will determine which emerging genome-editing and RNA-based candidates succeed.
