Аннотация

Most preventable deaths in low-income and middle-income countries occur not because effective interventions are unknown or unaffordable but because health systems fail to deliver these interventions reliably. Primary health care is essential for public health: prevention, early detection, and treatment can reach entire populations. Nearly 50 years after the Alma-Ata Declaration, few primary health-care systems effectively prevent, detect, and treat the leading killers. Two forces make now the moment to act: the abrupt 2025 decrease in development assistance and new technologies that might improve the ability to deliver and measure care at low cost. To close this implementation gap, primary health care should be prioritised, proactive, and accountable. Prioritised primary health care within a commitment to universal primary health care should fund the highest-impact actions first with transparent, nationally owned selection, and with core essential medicines free at the point of dispensing. Proactive primary health care should reach, track, and support patients before and between visits. Accountable primary health care should judge success by health outcomes with financing aligned with population health goals. Preventable deaths should be reviewed without blame as never-events feeding into improved prioritisation. Delivering these actions requires a skilled and supported workforce, a reliable supply of medicines and diagnostics, sustained financing, community trust, and data that can drive action. Decide first which outcomes matter most, identify the actions that produce them, strengthen the system those actions require, and judge investment by health gained.
