AIM: Electrodiagnostic testing (EDT) is a preferred invasive diagnostic approach because it provides the most accurate results for patients with carpal tunnel syndrome (CTS). However, researchers are now trying to develop noninvasive techniques for diagnosis. The aim of this study was to detect alterations associated with muscular atrophy and hardening in the thenar muscles of individuals with CTS using shear wave elastography (SWE).

MATERIALS AND METHODS: The study was conducted on 113 hands and wrists of 77 individuals who were diagnosed with CTS using EDT between February 2024 and June 2024. The control group consists of individuals in optimal health who have not been diagnosed with CTS or reported concerns. We conducted an SWE study on the median nerve and thenar muscles. The cross-sectional area (CSA) of the median nerve and thenar muscles was measured.

RESULTS: There were significant differences between the patient group and the control group in the median nerve CSA values and velocity values (0.15 ± 0.09 vs 0.09 ± 0.02 cm, and 3.4 ± 0.6 vs 2.8 ± 0.3 m/s, respectively; P<0.001). The patient group had significantly greater levels of thenar muscle velocity than the control group (2.96 ± 0.41 vs 2.70 ± 0.26 m/s; P<0.001). The CSA values of the opponens pollicis muscle in the patient group were considerably decreased compared that in to the control group (3.34 ± 0.89 vs 4.23 ± 0.93 cm, respectively; P<0.001) CONCLUSION: Our study demonstrated the feasibility of using simultaneous SWE on the thenar muscles in conjunction with median nerve SWE in patients with CTS.
