Therapeutic Communication as Autobiographical Memory Scaffolding: Bridging Developmental Science and Psychiatric Nursing Practice.

OBJECTIVE

To introduce the developmental and cross-cultural science of autobiographical memory to psychiatric nursing and argue that therapeutic communication functions as autobiographical memory scaffolding.

METHODS

This discussion article integrates three fields within the literature: elaborative reminiscing and autobiographical memory development (developmental psychology), overgeneral autobiographical memory (clinical psychology), and cultural variation in autobiographical recall (cross-cultural research). It applies the integration to psychiatric nursing assessment and intervention.

RESULTS

Nursing has not integrated autobiographical memory science into practice, assessment, or theoretical frameworks despite a robust evidence base. Therapeutic communication employs the same conversational behaviors that developmental research identifies as the mechanisms of memory scaffolding. Without a framework to distinguish clinically meaningful from culturally normative memory, the relationally oriented, temporally general narratives common among patients from collectivist backgrounds may be misread as pathology during psychiatric assessment.

CONCLUSIONS

Understanding therapeutic communication as autobiographical memory scaffolding enables psychiatric nurses to assess memory specificity as a transdiagnostic clinical marker, recognize culturally shaped recall as distinct from pathology, and intervene more deliberately to support narrative construction that is clinically functional and culturally congruent. A clinical vignette illustrates these implications. This framework transforms therapeutic communication from an intuitive relational practice into a deliberate, theoretically grounded cognitive intervention.
